St. Christopher’s Services are committed to offering holistic supports to the people who access and use our services. This includes Support for the things that are important for the person in order to ensure that they are safe and well, as well as the things that are important to the person to allow them to realise a good quality of life.
Each person who is supported by St. Christopher’s Services C.L.G is offered the opportunity to develop a Person Centred Plan.
Person Centred Planning is a way of supporting a person to find out about the things that they would like to achieve and enjoy now and in the future. It places the person at the centre of planning and encourages those that are supporting them to listen carefully to each person to find out what their interests and aspirations are.
A person’s circle of support is integral to the success of the plan, and each year an annual review of the Person-centred plan will be held. Here the person can tell or show those closest to them, what they would like to achieve. Goals and actions are set so that they can be supported to move closer to the life of their choosing.
Person centred planning in St. Christopher’s is informed by the HSE’s National Framework in Person Centred Planning which sets out a consistent approach to planning within services in Ireland. Staff are given guidance and training to equip them to best facilitate a person’s plan. Many staff have completed accredited training in Person Centred Focus to Disability with The Open Training College We are dedicated to setting a standard and embracing a culture of person centredness across the organisation.
